For all youth in 9-12 grade

Join us around the Parupsky camp fire as we roast hot dogs for dinner, play some games & just “hang out”. We will also decide our topic of study for the upcoming year with Hollie Holt-Woehl. Parent Volunteer Interest Form

There’s a number of ways you can be involved with the youth program at Family of God. You can be on the youth committee, which plans the yearly events; serve as a chaperone – both for evening or overnight events. You are needed – so please indicate your area of interest on this form and you WILL BE CALLED to check for your availability!  Click here for form.

Confirmation/Youth Covenant and Medical Information Form
For Youth in Grades 6 - 12

This form must be filled out along with this year's confirmation form.  It includes behavior guidelines and expectations, along with medical & emergency information for your child.  We will keep this on file at all times during confirmation and youth activities.  Friends and visitors attending youth activities will also be asked to complete a form.  The form will only need to be filled out one time during the school year.  Please let Kris Simon know if any contact or medical information changes during the year so records can be updated.  Click here to download form.
